Preparing for your Appointment

Thank you for selecting CHOC Urology Center. We look forward to working with our patients to diagnose and treat virtually all pediatric urology conditions in our state-of-the-art facility.

Referring providers, please learn more here about referring a patient to CHOC Urology. 

In order to make the first appointment more efficient and effective, we kindly ask the following:

  • Plan to arrive 15 minutes early for the appointment to complete necessary paperwork and get properly registered.
  • Please bring the child’s insurance card, completed Family and Patient Health History Form, a CD of images from any radiology studies pertaining to the child’s condition, and a voiding diary if there are concerns regarding urinary incontinence, frequency or urgency of urination.
  • Be sure that the child is well hydrated, as we often need a urine sample or other urine tests such as flow rates or post void residuals. Learn more about the common diagnostic procedures used by our staff.

It is important to note that we may have to reschedule patients who arrive more than 20 minutes late for their appointment. This allows our office to run as closel to on-time as possible. Because we provide surgical services in addition to our in-office practice, we occasionally fall significantly behind due to unexpected emergency surgeries. We apologize in advance for any inconvenience that this may cause. Please feel free to call our office the day of your appointment to verify that the center is running on schedule.
