CHOC Link is your connection to your child’s medical information at CHOC. It is a secure place to communicate with your child’s doctors and clinics. And it’s the most convenient way for you to manage your child’s health like you’ve always done – now with a free and easy-to-use tool.
How do I get access to CHOC Link if I am not already a member?
If you are not currently using CHOC Link, please speak to your health care team at your next appointment to sign up.
What can I do on CHOC Link?
As a parent or legal guardian, you can:
- View and print your child’s health issues, allergies and medications

Is CHOC Link secure?
Yes. CHOC takes great pride in ensuring that your health information is kept private and secure. Access to CHOC Link and the information within it is controlled by strict security measures.
Is there a fee to use CHOC Link?
No. CHOC Link is a free service available to CHOC patients and families.
Who can sign up for CHOC Link?
Parents and legal guardians of CHOC patients may sign up for CHOC Link. Patients 18 or older may create their own CHOC Link account. Note: Parents and legal guardians of children aged 12 to 17 will have limited CHOC Link access.
How do I sign up for CHOC Link?
At your next visit to a CHOC hospital or clinic, simply provide our staff with a valid email address. You will receive an email invitation to create your CHOC Link account and connect to your child’s records.
I didn’t receive my email invitation. What happened?
Check to see if the email is in your junk or spam folder. If not, please call 877-621-8014 to request another email invitation.
What do I do if my email invitation has expired?
If you have not created an account within 90 days of receiving an email invitation, the invitation will expire. Please email email@example.com to request another email invitation.
Can I have one CHOC Link account for my whole family?
Yes. One account can be used to manage the health of all children in a family.
Can I still access a hard copy of my child’s medical records?
Yes. While CHOC Link cannot display any of your child’s medical records after they turn 12, parents and legal guardians may still obtain a hard copy of a child’s records by calling our Health Information Management Department at 714-509-8460. Please note that certain information within the records may be withheld due to state laws.
Is CHOC Link available in Spanish?
Yes. CHOC Link is available in Spanish. After logging in, you will see a Spanish version link at the top of the page.
How do I access CHOC Link?
After you have created an account, log in to CHOC Link here or download the HealtheLife mobile application, available for iOS and Android operating systems.
CHOC Link is a secure online connection to your child’s medical information at CHOC. Parents and legal guardians can use this tool for children 0 to 11 years old. When your child turns 12, your access to CHOC Link changes.
What happens when my child turns 12 years old? When your child turns 12, your access to their CHOC Link information will stop. CHOC Link is restricted for parents and legal guardians of adolescents aged 12 and older due to state laws protecting minor confidentiality in certain health circumstances.
Why is CHOC Link being turned off for parents? Because of state laws, CHOC must limit what medical information parents can see for their children aged 12 and older. This is to protect a child’s privacy when it comes to sensitive medical topics, including sexual health and mental health. A patient might not share important health information with their provider if their medical information isn’t kept private.
Can I still have access to my child’s other health information? Yes, you will still be able to see your child’s medical information by calling our Health Information Department at 714-509-4368. The office is open 8 a.m. to 4:30 p.m., Monday through Friday.
How do I regain access to CHOC Link? We understand it’s important to remain involved with your child’s health needs, so we offer limited CHOC Link access to parents and legal guardians of children aged 12 to 17. To regain access to certain areas of CHOC Link, such as requesting medicine refills, messaging your child’s providers and scheduling appointments, you will need to create a new CHOC Link account. Please talk to our staff during your next visit, or call our Health Information Department at 714-509-4368.
What happens when my child turns 18 years old?
When your child turns 18, your access to their CHOC Link information will stop. Your child can request access to their own account.
How can I help my child understand their medical information? As an adolescent, your child is on their way to becoming an adult. With the help of their parents or guardians, as well as our staff, they will start taking small steps toward managing their own health. We encourage you to talk to your child about how to ask their doctor questions, make decisions and advocate for themselves. Our staff will help guide your family through this time. What is an appropriate reason to send a message to my child’s doctor?
Secure messaging is a way to discuss routine issues with your child’s doctor and ask general questions about a condition, procedure, medication or previous visit. The doctor may ask you to make an appointment if the issue is more suited for a clinic visit. Messaging should not be used if your child has an urgent need or is experiencing an emergency. If your child has a medical emergency, please call 911 or go to the nearest emergency room.
Can I attach documents and files to messages that I send to my child’s doctor?
Yes, the messaging feature allows you to include attachments.
How will I know when I have received a message from my child’s doctor?
You will receive an email notification when a new message is available to view.
When can I see lab test results in CHOC Link?
In most cases, your child’s lab results will be available to view within four business days.
What is the difference between refilling a prescription and renewing a prescription?
A prescription refill is done through your pharmacy. When a prescription has expired or its refills have run out, CHOC Link allows you to request a prescription renewal from your child’s physician.
Will my child’s primary care physician be able to access CHOC Link?
Primary care physicians will not be able to access CHOC Link directly. CHOC Link does allow you to send clinical documents to another physician who has a secure email address.
Why is some information missing from my child’s medical records in CHOC Link?
CHOC Link is designed to show a snapshot of the most useful information from your child’s medical records. To obtain a complete set of records, call the CHOC Children’s Health Information Management Department at (714) 509-8460. Please note that certain information within the records may be withheld due to state laws.
What should I do if I see a discrepancy or inaccurate information in my child’s medical record?
If you see a discrepancy or an error in your child’s information, please contact the CHOC Children’s Health Information Management Department at (714) 509-8460.
Can I use CHOC Link on a mobile device?
Yes, CHOC Link is accessible on all mobile devices.
What if I forget my password?
If you forget your password or your account becomes locked, please follow these steps to reset your password:
- Select “Forgot password?” from the login screen.
- Enter your username and submit.
- You will receive a message stating that an email has been sent to the address associated with your account.
- Check your email for the message regarding your password.
- Click the link contained in the email.
- The link will direct you to a screen where you will enter your username and answer your security questions. Once authenticated, you may enter a new password and confirm it.
I was logged out of CHOC Link. What happened?
Protecting the privacy and security of your information is our top priority. If your computer remains idle for more than 20 minutes while you are logged in to CHOC Link, you will be automatically logged out.
I’m having trouble with my account. What should I do?
Technical support is available 24/7. Please call our support line at (877) 621-8014 and select “patient portal” to talk to a representative. Spanish-speaking representatives are available Monday-Friday, 6 a.m.-3 p.m. PST.
