Is CHOC Link secure?
Yes. CHOC Children’s takes great pride in ensuring that your health information is kept private and secure. Access to CHOC Link and the information within it is controlled by strict security measures.
Is there a fee to use CHOC Link?
No. CHOC Link is a free service available to CHOC patients and families.
Who can sign up for CHOC Link?
Parents and legal guardians of CHOC patients who are age 11 or younger may sign up for CHOC Link. Children age 12 and older are not eligible at this time. Patients who are 18 or older may create their own CHOC Link account.
How do I sign up for CHOC Link?
At your next visit to a CHOC hospital or clinic, simply provide our staff with a valid email address. You will receive an email invitation to create your CHOC Link account and connect to your child’s records.
I didn’t receive my email invitation. What happened?
Check to see if the email is in your junk or spam folder. If not, please call (877) 621-8014 to request another email invitation.
What do I do if my email invitation has expired?
If you have not created an account within 90 days of receiving an email invitation, the invitation will expire. Please email choclink@choc.org to request another email invitation.
Can I have one CHOC Link account for my whole family?
Yes. One account can be used to manage the health of all children under 12 in a family.
What happens when my child turns 12?
State laws prevent us from giving parents and guardians online access to their children’s medical records once the minor turns 12 years old. On your child’s 12th birthday, your child’s information will no longer be accessible in your CHOC Link account. Any other children on your account will not be affected. When your child turns 18, they may create a CHOC Link account to access their own medical records.
Can I still access a hard copy of my child’s medical records?
Yes. While CHOC Link cannot display any of your child’s medical records after they turn 12, parents and legal guardians may still obtain a hard copy of a child’s records by calling our Health Information Management Department at (714) 509-8460. Please note that certain information within the records may be withheld due to state laws.
Is CHOC Link available in Spanish?
Yes, CHOC Link is available in Spanish. After log in, you will see a Spanish version link at the top of the page.
How do I access CHOC Link?
After you have created an account, log in to CHOC Link here or download the HealtheLife mobile application, available for iOS and Android operating systems.
