SEIZURE RECORD The
seizure record form is a valuable tool that can be utilized to
keep the school nurse, parents and the student’s physician
informed about the seizure activity of a student. The form may
be kept in the school office or in the teacher’s room which
ever is most appropriate. Periodically the form should be sent
or mailed home so that the parents are advised of their child’s
