Request for Price Quote or Estimate
The Business Office Revenue Audit Department is responsible for providing physician office staff, patients and third party payers with charge estimates for services rendered at CHOC. A simple form has been developed for requesting a price quote or estimate and is available online. These requests are provided in good faith and cover the hospital portion only and may be higher or lower depending upon the actual service provided.

Ordering a CHOC Children's Lab Coat
If you have an on-campus office with a CHOC Children's Cost Center, please visit CHOC Paws. In the Forms Bank, under EVS department, you will find the lab coat order form with instructions.
For providers who do not have a CHOC Children's Cost Center (or are offsite), there is a program available to directly purchase a lab coat from a private vendor. Please contact the Physician Relations manager at hmarcus@choc.org indicating you are interested in purchasing a lab coat and the details will be forwarded to you.
