CHOC Children’s Invites Your Input!

CHOC Children’s is requesting feedback from residents of Orange County about the health and social service needs of children in our community.  Please consider taking 10 minutes to complete our survey no later than Oct. 18, 2016.  Responses will help us assess the overall health of Orange County, and inform our priorities for meeting the healthcare needs of children.
